| 内容記述 | 《目的》本人が主体となって行う向精神薬の服薬に関する近年の日本での言説の変遷や動向について、文献研究から明らかにすることを目的とする。近年の動向として「減薬」「断薬」にも焦点を当てる。 《方法》対象とする文献は、国立情報学研究所の論文検索サイトCiNii Researchを用いて決定した。まず、「精神科×服薬×自己決定」で4本「精神科×減薬」で26本、「精神科×断薬」で37本、「精神障害×アドヒアランス」で15本が検索された。最終的には、研究論文45本、書籍3冊を対象とした。 《結果》対象文献から研究目的に関連した内容を以下の5点に整理し説明した。それは、「日本における減薬を推進するに至る経過と新たな動向」「向精神薬に関する多様な見解」「向精神薬の減薬に肯定的な医師の臨床経験」「当事者の主体性を尊重した服薬支援」「当事者の権利としての減薬・断薬」である。 《考察と結論》精神科治療における当事者主体の考え方は、リカバリー概念の具現でもある。当事者運動と結びついて向精神薬の減薬・断薬が、世界的には一つのムーブメントになっている。今後、日本でも、当事者の服薬や治療に対する主体性の喚起や、権利の回復に関する運動が活発になっていくだろう。精神科看護師や精神保健福祉士は、その活動にどのように関与し貢献できるのかを探求する必要がある。 |

| 内容記述 | Objective: This study aimed to conduct literature research to clarify the recent changes and trends in Japanese statements concerning psychotropic drug administration conducted mainly by people with mental disorders. Recent trends will also focus on “reducing drugs” and “cutting off drugs.” Method: The literature targeted for the method was determined using CiNii Research, a paper search site of the National Institute of Informatics. First, four literature were searched for “psychiatry × medication × self-determination,” 26 for “psychiatry × drug reduction,” 37 for psychiatric × drug reduction, and 15 for “mental disorder adherence.” Finally, this review included 45 research papers and 3 books. Result: The contents associated with the research purpose in the target literature were summarized and explained in five points, including “progress and new trends leading to drug reductions in Japan,” “diverse views on psychotropic onions,” “positive clinical experience of doctors in reducing drugs,” “support for taking drugs with respect for the parties’ independence,” and “reduction/disruption as a right.” Conclusions: The concept of the parties involved in psychiatric treatment is an embodiment of the concept of “Recovery.” Reducing and cutting off psychotropic drugs has become a worldwide act in conjunction with the party movement. In the future, more activities will be conducted in Japan to encourage people to take medicines and treatments independently and to restore their rights. Psychiatric nurses and mental health social workers need to explore how they can engage and contribute to their activities. |
