@article{oai:mejiro.repo.nii.ac.jp:02000058, author = {坂崎, 弘幸 and 瀧元, 美和 and SAKAZAKI, Hiroyuki and TAKIMOTO, Miwa}, issue = {17}, journal = {目白大学健康科学研究, Mejiro Journal of Health Care Sciences}, month = {Mar}, note = {pdf, 《目的》リッカムプログラム(Lidcombe Program、以下LP)は小児吃音への介入方法として有効性が示されているが、本邦では導入の待機児が多い。一方、環境調整法は保護者に求められる手技にLPとの共通点が多い。そのため、LPの導入待機中に環境調整法を行うと、保護者はLPに必要な技術を一部習得した状態でLPを開始できる可能性がある。そこで、本研究ではLPの導入待機期間中に経過観察ではなく環境調整法が行われた場合のLPへの影響について検討することを目的とした。 《方法》対象期間内にLP指導を受けた吃音児のカルテを後方視的に分析した。LP導入前に環境調整法の介入がある症例をⅠ群、Ⅰ群以外の症例をⅡ群としLP導入後の吃音症状の経過を調査した。また両群から1名ずつ基礎情報が類似した症例を抽出し比較した。 《結果》Ⅰ群に該当したのは3例であり、LP導入後に吃症状は一時的に悪化した症例もあるが改善傾向が認められた。一方、Ⅱ群に該当した3例では改善傾向は認められなかった。 《結論》LP導入前に環境調整法を行うと保護者はLPに必要な手技の一部をLP導入前に習得しており、LP導入後の訓練に好影響を与える可能性が高いと考えられた。, Objective: The Lidcombe Program (LP), as an intervention for childhood stuttering, has demonstrated effectiveness. However, a substantial number of children who stutter await implementation in Japan. Conversely, environmental modification techniques share commonalities with LP in terms of the skills required by parents. Therefore, implementing environmental modification interventions during the LP waiting period may enable parents to acquire some of the necessary skills for LP. This study aimed to investigate the effects of environmental modification interventions during the LP waiting period, rather than mere observation, on the subsequent LP implementation. Methods: This study retrospectively analyzed the medical records of children who underwent LP during the study period. Cases with environmental modification intervention before LP introduction were classified as group I, whereas those without such intervention were classified as group II. The progression of stuttering symptoms after the LP introduction was investigated. Additionally, one case with similar baseline information was extracted from each group for comparison. Results: Group I consisted of three cases, and an overall improvement trend was observed despite instances of temporary worsening of stuttering symptoms after LP introduction. In contrast, group II, comprising three cases, demonstrated no significant improvement in stuttering symptoms. Conclusion: Conducting environmental modification interventions before LP introduction may cause parents to acquire some of the necessary skills for LP, thereby potentially exerting a positive influence on training after LP introduction.}, pages = {121--128}, title = {小児吃音に対するリッカムプログラム導入前に環境調整法を行うことが訓練効果に及ぼす影響}, year = {2024} }
