{"created":"2023-06-19T07:35:53.860527+00:00","id":1723,"links":{},"metadata":{"_buckets":{"deposit":"db59dcb0-0b82-4eb1-9901-a5007d4492b0"},"_deposit":{"created_by":10,"id":"1723","owners":[10],"pid":{"revision_id":0,"type":"depid","value":"1723"},"status":"published"},"_oai":{"id":"oai:mejiro.repo.nii.ac.jp:00001723","sets":["1:19:139"]},"author_link":["4253","4250","4252","4251"],"item_10002_biblio_info_7":{"attribute_name":"書誌情報","attribute_value_mlt":[{"bibliographicIssueDates":{"bibliographicIssueDate":"2021-03-31","bibliographicIssueDateType":"Issued"},"bibliographicIssueNumber":"14","bibliographicPageEnd":"42","bibliographicPageStart":"35","bibliographic_titles":[{"bibliographic_title":"目白大学健康科学研究"},{"bibliographic_title":"Mejiro Journal of Health Care Sciences","bibliographic_titleLang":"en"}]}]},"item_10002_description_19":{"attribute_name":"フォーマット","attribute_value_mlt":[{"subitem_description":"pdf","subitem_description_type":"Other"}]},"item_10002_description_5":{"attribute_name":"抄録","attribute_value_mlt":[{"subitem_description":"《目的》本研究は、地域包括ケアシステムにおける訪問看護の連携の現状と課題を、退院支援の多職種連携に焦点を当てて文献検討を行い、連携の障壁や対処方法の示唆を得ることである。\n《方法》医学中央雑誌により、①「訪問看護and連携and原著」1537件を概観し②「訪問看護and連携and原著and2015-2020 and地域包括ケアシステム」から47件を抽出し、在宅看護の研究者で精選したキーワード:「退院支援」「リハビリテーション」「終末期」「精神疾患」「認知症」「がん」などについて文献を分類した。そのうち最も数が多く、地域包括ケアシステムの中で特に重要となる「退院支援」について検討した。\n《結果》2000年の介護保険制度の施行後、①は著しく増加していた。さらに②の文献47件のうち「退院支援」に関連した文献は、14件であった。事例3件、アンケートやインタビュー調査8件、文献検討1件、看護師の研修の評価2件であった。病棟看護師から見て退院困難な事例が多職種連携により自宅に帰る事が可能となっていた。また、病院の退院支援看護師や訪問看護師には意識の違いがあり、訪問看護師は対象者の生活を支える為に日常生活の情報を必要としていた。また、多職種の連携推進には研修が有効であった。\n《結論》多側面の課題を抱える患者の退院に多職種連携が有効であるが、課題として、病院看護師と訪問看護師、サービスを提供するスタッフと行政・福祉職の認識に違いがあり連携の障壁となっていた。この認識の違いは同行訪問や研修などの教育によって変化した。そのため、多職種連携の推進には研修や教育が有効であることが示唆された。","subitem_description_type":"Abstract"},{"subitem_description":"Objective: This study examined the current status and issues associated with home-visit nursing cooperation in a community-based comprehensive care system, focusing on multidisciplinary cooperation for discharge support and obtaining suggestions for barriers to cooperation and coping methods. \nMethod: Referring to the Central Medical Journal Database, (1) we reviewed 1,537 articles including the following keywords: home-visit nursing, collaboration, and original work; and (2) extracted 47 articles from them on the basis of the following keywords: 2015-2020 and community-based comprehensive care system. Keywords selected during the discussion were: \"Discharge support (transitional period),\" \"Rehabilitation,\" \"Recovery Period,\" \"Terminal Stage,\" \"Mental Illness,\" \"Dementia,\" and \"Cancer,\" among others. The contents of the paper on \"Discharge Support\", which is important in the comprehensive community care system, are summarized in a table. \nResults: Post the implementation of the long-term care insurance system in 2000, the number of original articles concerned with home-visit nursing and collaboration has increased significantly. Furthermore, of the 47 articles selected in (2) above, 14 were related to \"discharge support.\" There were three cases, eight questionnaires and interview surveys, one literature review, and two evaluations on nurse training. \nWe also found the following. First, multidisciplinary collaboration enabled patients to return home even in cases where ward nurses had not believed that they could. Second, there was a difference in consciousness between discharge support nurses and visiting nurses in hospitals; visiting nurses required medical assessment data on subjects on a daily basis to support their lives. In addition, training was effective in promoting collaboration among multiple occupations. \nConclusion: Multidisciplinary cooperation is effective for the discharge of patients with multifaceted issues; however, there are differences in their views of multidisciplinary cooperation among hospital nurses and visiting nurses, staff providing services, and administrative/welfare workers. This difference changed, if not dissolved, with education such as accompanying visits and training.
